  1. How do I get started?

    In order to choose a California Cryobank donor for any treatment in Australia, you must first be registered with Monash IVF or one of their associated clinics. The first step is to contact a Monash IVF Nurse in Australia on 1800 628 533. Monash IVF Nurses are available to answer any questions you may have.

    Once you have been registered and approved by Monash IVF, you are ready to choose your preferred California Cryobank donor.

  2. How do I search donors?

    Click on the Donor Search button and you will be taken to California Cryobank’s catalogue of available donors. It is important to note that only “Australia Compliant” donors are available for shipment to, and treatment in, Australia due to the strict regulations on the use of donor sperm.

    If you wish to access extended donor profile information, you can contact your Monash IVF Nurse who will give you a special promotion code that will allow free access to Level 3 donor information. Level 3 donor information will enable you to access additional donor photos, listen to donor conversations and view extended details

  3. How do I choose a donor?

    Please ensure you are registered and approved to receive donor sperm in Australia through Monash IVF. If you are not yet registered and approved, please contact a Monash IVF Nurse on 1800 628 533. Once registered and approved, you can then register on the California Cryobank website for a free account.

    Once you are logged-in, you can save your search criteria and favourite California Cryobank Australian-compliant donors. You can also view Childhood and Adult Photos and download Donor Profiles, Medical Histories, Genetic Test Summaries, Donor Personal Essays and Staff Impressions. Choose 2 x donors, a preferred donor and a back up, just in case your preferred donor is unavailable.

    Once you have your preferred selection, and you are registered and approved, you can contact your Monash IVF representative who will assist you in completing a Donor Reservation Form.

    Remember, if you wish to access extended donor profile information, you can contact your Monash IVF representative who will give you a special promotion code that will allow you free access to Level 3 donor information.

  4. How do I reserve donor sperm vials?

    Before you can make a reservation for donor sperm vials, you need to ensure that you have completed all required paperwork. Your Monash IVF representative should be able to help with this process. The documents required to be completed are:

    • Monash IVF’s “Donor Reservation” Form
    • California Cryobank’s “A2” and “A3” forms (online)
    • California Cryobank’s “International Shipping Agreement”

    To avoid disappointment and delays, please contact your Monash IVF representative to make sure you have everything ready in order to reserve the donor your vials you wish to order. All vials reserved for treatment in Australia must be ordered through Monash IVF.

  5. How do I obtain a Level 3 Donor Information Subscription?

    Monash IVF patients are entitled to a free access code to a Level 3 donor information subscription (a USD 250 value).

    Simply contact your Monash IVF representative who will provide you with a special promotion code that will allow you free access to Level 3 extended donor information. Level 3 donor access will enable you review additional donor photos, listen to donor conversations and extended profile information.
